It depends. I can recognise layouts like Orlandu's, Metal_Man's and other large posters, but if it's somebody I don't recognise, then it's the username I go from.

Layouts generally since most people stick with the same style. But that's if I'm just glancing quickly... usually I look at the name first.

Usually by post layout as well. As most active people use them, it's not too hard to "recognize" users like Orlandu, Acmlm and others. For people with no layouts simply by user name, as avatars may change often. There are people who use the same user pictures over time, like myself (although being not too active), but as you only see the user names on /newreply.php, I go with names anyway.

It really depends. I think I identify everyone by a different combination of things, but the thing is there's no real idea of a person there, just a collection of data. Recollection of different pieces of information can be triggered by different aspects of a post. I usually try to match the layout (or avatar) to a face (or avatar) first, and then match salient typing quirks to interesting quotes from past posts. If I got to know anyone better I would probably have 'real' personality information floating around too, as gleaned from conversations, but honestly without being able to see your faces in motion or hear your voices change with your emotions I can't conceive of any of you as real people.

General knowledge of HTML and/or CSS is a must. You put the start of your layout in the header (edit profile) and then the closing tags in your signature.
