There are a few things wrong here.
First off, you are editing at the wrong offset.
Secondly, it seems Tile Molester has set all of the gray area to be opaque. I don't know if Tile Molester has any functions to edit the alpha channel.
Third and most importantly,
Tile Molester has serious bugs in all 16, 24, and 32 bit color modes. Do not use Tile Molester. Textures do not appear in the editor the same way they appear in-game, and
editing or importing can cause corruption.
Let's take a look at the first image you posted.
Notice the strange blue/yellow haze around the original hand image, and how your replacement has no such haze. Now let's look at your replacement hand icon in the game.
There's the haze again, with the colors reversed. These are the most visible effects of the bugs. However,
non-texture data can be corrupted, so do not use Tile Molester.
