I'm creating a sm64 hack with ny brother and we got that problem... so every tine when we fail a level, we get portet to the bob omb battlefield exit. When we win the level (for exame in cool cool mountain) we come out of the right (cool cool mountain) picture. But if we fail the level we're ported to the bob omb battlefield exit (fallling out of it, after being desth).... we can't even fix this bug .. is there any way to do this.!??!
