Blood Clot Boy isn't that hard either.

As for Brimstone boy, 4-2 is a run killer(Oh hey, i got killed by NOTHING)

Dr. Fetus Boy is on my list of later stuff.

I'm mostly aiming for Maggot Boy while practicing for Missile Boy.

So I have a question; when it says "no deaths" does that mean I start at, say 1-1, and I have to go to 1-24 and then beat the boss without dying, or do i just have to exit to map every time I die?

You need to beat all 20 levels in the world (either light or dark depending on which achievement you're going for). It can be in any order, but you must beat all 20. You can't die after you finish the first level and you can change character at any time (although if you use Steve or Meat Ninja it doesn't count).
