Scroll wheel usually. What really annoys me is that some Windows applications are put together so poorly, that they use 'panels' where the scroll wheel doesn't work, and you're forced to have to click and/or drag 'n drop the scrollbar.

Depends. I mostly use the wheel on web pages, or home/end and page up/down. When reading a text file I often use the arrow keys. In code I usually use page up/down, bookmarks, or jump directly to a line number. In image viewers/editors I drag the middle button to pan around (all sane image tools support this).

I use the middle-click function of web browsers or drag the scroll bar on occasion when I need to skip through a page quickly looking for something (and can't just use the search tool).

Alternation between the scroll wheel and keyboard shortcuts if I'm too lazy to move my hand about a foot to reach the mouse.

Usually Page Up and Page Down. Sometimes the scroll wheel if I need to use the mouse anywhere (to submit something or add checkmarks, for example).

It really depends on which computer I am using. When I am on my mac, the touch pad is a must. It is so easy to use and unlike PC laptops it is actually accurate. When I am on my PC I use my wonderful scroll-wheel/middle click mouse ^^
