Oh, it's happened before. There is only 1 Lose A Turn space and 2 Bankrupt spaces in rounds 3 and later (1 Lose A Turn and 1 Bankrupt before that), but there was an episode in which the wheel simply would not stop landing on them. Also, it has happened at least twice that people got through 2 episodes without having solved a single puzzle.
