Before I used a gamepad, I'd use Right Ctrl and Right Alt for B+A on a NES game. SNES was even better... I used Insert for Y, Delete for B, Home for X, End for A, Page Up for L and Page Down for R. XD

For GB/A, I'm using:
A = Z
B = X
L = A
R = S
start = enter
select = left control
up = P
left = L
down = ;
right = '
I've been using arrow keys since it was the default on VBA, but I switched because of being unable to hold B and move diagonally at the same time. Annoying. Now I find this layout better.
Genesis/Mega Drive is similar:
ABC = ZXC
XYZ = ASD

When playing multi on one computer (a laptop, so no numpad):
Player 1:
D-pad = arrow keys
A = ,
B = .
(C) = /
L(X) = L
R(Y) = ;
(Z) = /
start = enter
select = control or shift
Player 2:
D-pad: wasd
A = \
B = shift
(C) = control
L = Q
R = E
start = caps lock
select = tab

It depends a lot on the game we're playing, though.

For SNES (which is mostly what I play) I use this set up:

Arrow Keys = D-Pad
Start = Enter
Select = Right Shift
W = X
A = Y
S = B
D = A
Q = L
E = R

Still need a better layout for Super Metroid though... it really has horrible controller set up. D:

I'd like to get a Wii Classic Controller USB adaptor though. (I saw it on some Chinese manufacturing site, heh. No price listed. Nor was there an order form. Exactly how that site expected to sell anything I'll never know.)

Here's my setup for different things.

NES/any platformer:
Arrow keys = move (D-pad)
Z = shoot/run (B)
X = jump (A)
G = select
H = start

SNES/GBA:
Arrow keys = D-pad
A = X
Z = Y
S = A
X = B (Sometimes I'll swap around these four in order to get the Z/X config for platformers, especially for GBA games)
Q = L
W = R
G = select
H = start

N64/GC/Wii:
Game specific, but I usually use these:
Arrow keys = analogue stick (or D-pad, depends on the game's main movement)
Z = run
X = jump
C/V = extra buttons
G = select
H = start
J = Home button (on Wii)

I can't remember all my game specific configs, but here's the only one I can remember:
NSMB Wii:
Arrow keys = D-pad
Z = run (1)
X = jump (2)
A/S = tilt Wiimote (left, right)
D = shake
G = - (World Select)
H = + (Pause)

Last night I was playing a SMB1 hack and kept pressing D to shake the Wiimote and lower my gravity every time I jumped off something - despite the lack of that in SMB. I've been playing NSMBWii too much.

SNES9x
D-pad = Arrow keys
Start = Space
Select = Enter
B = C
A =V
Y = X
X= D
L = A
R= S
