I was enjoying POWDER and had died quite a few times before I got really far and ended up using some haunted weapon that killed me the second I equipped it. I never picked it up again.

I don't know if it compares to being killed by an incompetent masseur, but I have been known to manage blowing myself up with a thermonuclear bomb in Doom: The Roguelike a couple of times.

I've died in the town shortly after starting a new game in UMoria/Angband. Those dumb Squint-eyed rogues and Mean-looking mercenaries are tough when you're level 1 and you only have your starting gear.

Just about any death in NetHack is a stupid death. If you aren't completely paranoid about everything you'll die. If you aren't a total metagamer, you'll die. Just by living you have a risk of dying somehow.

Example: Don't eat every egg you see. One of them might be a cockatrice egg.

I'm sure I died once eating one but I had gloves on, so...

Falling down the stairs while wielding a cockatrice corpse as well. So many fun ways to turn to stone.
