Let's see... a shaft painted with normal brush, made it to look like a proper shape, bewel and emboss, shadows, veins, overlay on them, red slow glow, white blobs, bewel and emboss, opacity, shadows.

Liquify on the balls, bloat tool, some dodge and blur.

Voila.
